Safe space: the Healthcare Safety Investigation Branch

Sean Weaver and Cassandra Cameron from the HSIB ask what the introduction of 'safe space' means for safety investigations in English healthcare.

It may take some time for the public and media to trust that 'safe spaces' for healthcare professionals and investigations will achieve greater transparency and insight about why harm happens during healthcare, rather than less.

Cassandra Cameron

Cassandra is head of policy and strategy at HSIB since January 2019, building the organisation's external relationships and influence in England and internationally. She has extensive health policy experience from previous work at NHS Providers, and broader policy and international relations roles in the Australian diplomatic missions to London and Ottawa.

Dr Sean Weaver
Deputy medical director – Healthcare Safety Investigation Branch (HSIB)

Sean splits his time between being a consultant gastroenterologist at University Hospitals Dorset and being deputy medical director at the Healthcare Safety Investigation Branch - HSIB. He trained in Oxford, Cambridge, Sydney and Bath where he obtained a PhD in molecular pharmacology. He has been clinical director and subsequently associate medical director for safety and quality, and was national lead for the Royal College of Physicians Inflammatory Bowel Disease Quality Improvement programme. At HSIB he has co-authored the national learning report on Never Events, published on systems approach to patient safety investigation and has a particular interest in safety management systems.
