Find out as much as you can about the purpose of the report. Is it for the coroner/procurator fiscal, a serious untoward incident investigation, a police or GMC investigation?
Make sure you have a copy of the relevant clinical records and any other documents you'll need to write your report or statement.
Check your report is factually accurate and detailed. Make clear what is based on the clinical records and what is based on your recollection of events.
Send your draft statement, the relevant clinical records, and any associated correspondence (removing patient identifiers) to the MDU.
