1. Create a case and have your membership number and password ready.

2. Complete the form within 14 days.

3. Review your questionnaire.

4. Submit your form.

What happens after I've submitted my case?

  • We will send you an email and provide you with a case ID.

  • We may ask you to submit further documentation to support your case.

  • A dedicated case handler will be allocated to your case.

  • Once your case handler has been assigned, they will remain in touch throughout your case.
